Hearing Aid Bank

District 23-A’s Speech and Hearing Mission

The mission of the Hearing Aid Bank, established by the Lions Clubs of District 23A is to make affordable hearing aids more accessible to hearing impaired children and adults, who present a financial need or when there are special circumstances that need considering. By playing a role in improving the quality of life for the hearing impaired, we advance the Lions Clubs purpose and motto, “WE SERVE.”

Success of the Hearing Aid bank depends on the availability of usable hearing aids that can be reconditioned and the generosity of the Lions Clubs of District 23A through their yearly contributions. All Clubs are encouraged to support the Hearing Aid Bank by collecting hearing aids no longer in use, and continuing yearly contributions to the Speech and Hearing Program. This benefits both the Lions Hearing Aid Bank and the Communication Disorders Program at Yale New Haven Hospital. (link: http://www.ynhh.org/index.html).

We Collect and Recycle Used Hearing Aids

•        District 23-A Lions Clubs collect and recycle used hearing aids.

•        Used behind-the-ear style hearing aids are reconditioned and issued to eligible hearing impaired persons applicants.

•        Used in-the-ear style hearing aids that cannot be reconditioned are desirable as well because they can be used to obtain credit towards repairs made on the behind-the-ear style hearing aids.

•        A letter of acknowledgement for receipt of donated hearing aids for tax purposes is available.  For clubs, the form may be obtained by contacting either of the co-chairs above.
Include your name and address/or email address and your club name.

Guidelines for Use of Hearing Aid Bank for Applicants

Responsibility of the Applicant

•        Applicant has had a hearing evaluation by a licensed audiologist in the last six months and a printed audiogram of the results.*

•        Applicant completes and returns the application form with the audiogram to the address on the form. The form is available in Adobe Reader format HERE; in MS Word format HERE.

1.      Completed applications are returned to the Speech & Hearing Co-Chairs for review and processing. The return address is:

•        If eligible for the Lions Hearing Aid Bank program, the applicant will be asked to contribute $50.00 towards the cost of the hearing aids, and any amount family members are able to donate on the applicants behalf.

 

*If the applicant has not had a recent hearing evaluation, the Lions Clubs can offer the audiological services at the Southern Connecticut State University, Department of Communication Disorders (link: http://www.southernct.edu/communicationdisorders/thecenterforcommunicationdisorders/) 203) 392-5954. A sliding fee scale will be provided to the applicants, which will be about $50.00.

 

Responsibility of the Lions Clubs of District 23A

•        Receive requests for hearing aids, and provide applicants with application form. Explain the process to the applicant or refer the applicant to the District Speech and Hearing CoChairs.

•        Contribute what the Clubs Lion Share can be for the cost of the hearing aids and the molds.

•        Follow-up with the applicant until the hearing aids are delivered.

•        Collect behind the ear hearing aids that can be used for reconditioning and in the ear hearing aids for credit towards the cost of reconditioning hearing aids for use in the hearing aid bank.

 

Responsibility of District 23A Speech & Hearing

•        Receive and review applications for hearing aids and determine from the information provided and with any additional input from the sponsoring club, the applicant’s eligibility.

•        Forward applications of eligible applicants to Southern Connecticut State University where the audiologist selects the appropriate hearing aids for the applicant. The hearing aids are sent out for reconditioning, and returned to the audiologist. The applicant is contacted and an appointment is made to fit the hearing aids.

•        Track the process of each application to insure a timely process.

•        Notify the District Governor when additional resources are needed to meet demands.

  1. Your Lions Club receives a request for a hearing aid from someone in the community, or a District 23A Speech & Hearing CoChair informs the Club of a request.
  1. The person making a request for a hearing aid must complete an application form. Application forms can be down loaded from the District 23A web site. The form is available in Adobe Reader format HERE; in MS Word format HERE.
  1. Completed applications are returned to the Speech & Hearing CoChairs for review and processing. The return address is:

  1. Application forms are sent to the audiologist at Southern Connecticut State University, Department of Communication Disorders. The audiologist selects the appropriate behind the ear hearing aid from the hearing aid bank for the applicant.
  1. The hearing aids are sent for reconditioning and then returned to the audiologist at Southern Connecticut State University.
  1. The applicant is contacted by Southern Connecticut State University for an appointment to have ear molds made and fitting of the hearing aids.
